Signed FASTag application form provided by issuer bank which customers need to fill and submit required documents to the bank. Along with it, they need to submit the following documents to apply for FASTag based on their category.

All required documents for FASTag need to be in the name of the vehicle owner only. If the vehicle owner is not present at the time of application, driver will have to submit his photo ID proof as well.
A valid driving license would be sufficient for Address & ID proof.
