Write a request letter to the branch manager of the bank informing about the death of the account holder and showing intension to get the account proceeds

Submit following documents with the request letter,

• Copy of the account holder’s death certificate
• Duly filed "Claim Form" which can be collected from the branch of the bank
• Nominee's Identification proof of the nominee showing a relationship with the deceased
• Photograph of the nominee
