Details Required to be filled in an Aadhaar Enrolment Form

The Aadhaar Enrolment Form consists of the following fields:

  1. Pre-Enrolment ID
  2. NPR Receipt/TIN Number
  3. Full Name
  4. Gender
  5. Age
  6. Address
  7. Email
  8. Mobile Number
  9. Details of Name & EID (or Aadhaar Number) of Father/ Mother/ Guardian (mandatory for children below 5 years of age)
  10. Details of Name & EID (or Aadhaar Number) of Husband/ Wife (Not mandatory)
  11. Verification Type: (a) Document Based (b) Introducer Based (c) Head of Family Based
  12. For Document Based: Details of PoI, PoA, DoB & PoR
  13. For Introducer Based: EID/ Aadhaar Number of Introducer
  14. For Head of Family Based: EID/ Aadhaar Number of Head of Family (HoF)
  15. Name, Signature & Confirmation of Introducer & Head of Family

How to Fill the Aadhaar Enrolment Form?

Aadhaar Enrolment Form needs to be filled up while applying for Aadhaar Card. Following is the step by step process of filling the Aadhaar Enrolment Form:

  • Field 1: Pre-Enrolment ID: In this field, you need to mention pre-enrolment ID or the acknowledgement number received by you after online application of Aadhaar.
  • Field 2: NPR Receipt/TIN Number: In this field, you can fill up National Population Register (NPR) survey slip or a Tax Identification Number (TIN), if you have it however, it is not mandatory.
  • Field 3: Full Name: In this field, you mention first name, middle name (if any) and last name (surname). Please remember that this should exactly match with the full name mentioned on proof of identity being submitted by you.
  • Field 4: Gender: You have to tick one of the options from Male/Female/Transgender.
  • Field 5: Age: You can either mention your age in years or your Date of Birth in "DDMMYYYY" format. Then there are two more boxes Verfied and Declared.
    • If you are submitting proof of your Date of Birth (DOB), you have to tick on the 'Verified' box.
    • If you do not have any proof for your DOB, tick on the 'Declared' box.
  • Field 6: Address: You have to mention your full address which should be same as per proof of address being provided by you. Please mention the address accurately and completely as your Aadhaar card will be sent to this address only. There are separate columns to identify House/Building number, Street number, Landmark, Area/Locality, City, Post office, District, Sub-district and State for more clarity.
  • Email: Mention your email id here. This will become your registered email id for further communication.
  • Mobile Number: Mention your Mobile Number in this field. This will become your registered Mobile Number for further communication.
  • Field 7: Details of Name & EID (or Aadhaar Number) of Father/ Mother/ Guardian:
    • It is mandatory to provide father/mother/guardian details with their Aadhaar or EID number for child aged below 5 years.
    • If the resident is not having a Proof of Identity & using the Head of the Family identity for enrolment, it is mandatory to provide Head of the family's details with his/her Aadhaar or EID number. Please bring the original Proof of Relationship (POR) document.
  • Details of Name & EID (or Aadhaar Number) of Husband/ Wife: It is not mandatory
  • Verification Type: (a) Document Based (b) Introducer Based (c) Head of Family Based
  • Field 8: For Document Based: Details of PoI, PoA, DoB & PoR: Mention the name of Documents for PoI and PoA. If proof of Date of Birth is available, then write the name of Date of Birth document. If the resident is not holding a Proof of Identity & using the Head of Family based enrolment, then write the name of Proof of Relationship document.
  • Field 9 - Part 1: For Introducer Based: Resident who does not have POI and POA may get enrolled through an Introducer. In such cases, EID/ Aadhaar Number of Introducer is to be mentioned in this column.
  • Field 9 - Part 2: For Head of Family Based: Resident who does not have POI and POA may get enrolled through Head of Family. In such cases, EID/ Aadhaar Number of Head of Family (HoF) is to be mentioned in this column.
  • Name, Signature & Confirmation of Introducer & Head of Family
  • Applicant's Signature/ Thumbprint with Disclosure
  • Verifier's Stamp and Signature
  • Date and time of enrolment

Important Points regarding Filling the Aadhaar Enrolment Form

  1. Aadhaar Enrolment Form must be filled up in capital letters (uppercase letters) only
  2. Fill in the details in the form such as your Full Name, Address & DoB as per the details mentioned in the proofs being submitted by you
  3. One of the parents has to mandatorily submit his/ her Aadhaar details for enrolling Aadhaar for child aged below 5 years
  4. Don't forget to mention mobile number and email id
  5. No biometric is capture for enrolling Aadhaar for child aged below 5 years
  6. Corrections in Aadhaar Enrolment Form can be made within 96 hours of enrolment free of cost
